Please note that without your agreement to the following I will be

unable to offer my help and support as your healer and therapist:

I agree that my full name (current and any previous), date of birth, the area in which I live, conditions diagnosed by medical practitioners, treatments received previously and therapeutic approaches used by Helen Jameson, may all be used in informing potential clients of the outcomes, and medical practitioners off the efficacy of Helen’s treatments.

Helen may use information I give her regarding my health on social media, including, but not limited to, recorded interviews, live demonstrations, written testimonials and verbal reports noted concurrently by her.

I understand that she will respect my privacy and my feelings at all times and that this is purely to extend the knowledge and understanding of medical practitioners, the general public and her clients of the potential / experienced benefits of the therapies she offers.

She will maintain on computer only basic personal information - your email address, phone number and other information collected on her website “Contact Form.”

Other information of a personal nature will be retained in hard copy, in a locked cabinet.

You may have access to any information you have given her by personal request, upon payment of any expenses incurred “at cost.”
